Job summary

West-Coast-Wound- seeks a skilled Physical Therapist to deliver home-based PT services across the Tucson region. You will evaluate patients in their homes, develop individualized plans of care, and provide therapy to improve mobility, safety, and independence.

This role requires strong clinical judgment and excellent communication with patients and caregivers. The position is a full-time, 1099 independent contractor arrangement with a four-day, 10-hour workweek and travel to patients’ homes

Qualifications

  • Arizona Physical Therapist license in good standing.
  • Graduate of an accredited Physical Therapy program.
  • Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
  • Excellent patient assessment and documentation skills.

Responsibilities

Job description

Overview

We are seeking a skilled, compassionate, and dependable Physical Therapist (PT) to provide high-quality physical therapy services to patients in their homes throughout the Tucson region.

The Physical Therapist will travel to patients' homes to perform comprehensive evaluations, develop individualized plans of care, provide skilled therapeutic interventions, monitor patient progress, and educate patients and caregivers on strategies to improve mobility, function, safety, and independence.

The ideal candidate is comfortable working independently in a home-based environment, has strong clinical judgment and communication skills, and is passionate about delivering patient-centered care.

This is a 1099 independent contractor position.

Duties and Responsibilities
  • Travel to patients' homes throughout the assigned service area to provide physical therapy services.
  • Perform comprehensive physical therapy evaluations in the patient's home environment.
  • Assess strength, range of motion, balance, gait, mobility, coordination, endurance, pain, functional limitations, and ability to safely perform activities of daily living.
  • Evaluate the patient's home environment and identify potential safety and fall risks that may impact mobility and independence.
  • Develop individualized plans of care based on evaluation findings, physician orders/referrals, patient needs, clinical judgement, and functional goals.
  • Establish measurable short-term and long-term functional goals.
  • Provide skilled physical therapy interventions appropriate to the patient's diagnosis and functional limitations, including:
    • Therapeutic exercises
    • Therapeutic activities
    • Neuromuscular re-education
    • Gait and mobility training
    • Balance and coordination training
    • Functional strengthening
    • Manual therapy when clinically appropriate
    • Transfer training
    • Fall prevention and safety training
  • Train patients in the safe use of assistive devices such as walkers, canes, and other mobility equipment.
  • Develop and instruct patients in individualized home exercise programs.
  • Educate patients and caregivers regarding diagnosis, treatment, exercises, mobility, safety, fall prevention, and strategies to promote independence.
  • Monitor patient response to treatment and make appropriate clinical modifications to the plan of care.
  • Perform reassessments and communicate changes in patient condition or functional status as appropriate.
  • Coordinate care with physicians, nurses, caregivers, physical therapist assistants, occupational therapists, and other healthcare professionals.
  • Communicate effectively with referring providers regarding patient progress, concerns, and recommendations.
  • Complete accurate, timely, and thorough documentation for evaluations, daily treatment visits, reassessments, progress reports, and discharge summaries.
  • Maintain compliance with applicable federal and state regulations, professional standards, payer requirements, and company policies.
  • Recognize changes in patient condition that may require physician notification, additional medical evaluation, or modification of treatment.
  • Maintain patient confidentiality and appropriate professional boundaries at all times.
  • Provide compassionate, respectful, and professional care to patients and their families in the home setting.
Requirements
  • Current and valid Arizona Physical Therapist license in good standing.
  • Graduate of an accredited Physical Therapy program.
  • Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.
  • Strong clinical reasoning and patient assessment skills.
  • Ability to independently perform comprehensive PT evaluations and develop appropriate plans of care.
  • Experience treating patients with orthopedic, musculoskeletal, neurological, balance, gait, weakness, mobility, and/or functional limitations preferred.
  • Strong knowledge of therapeutic exercise, gait training, balance training, functional mobility, neuromuscular re-education, and fall prevention.
  • Excellent patient assessment and acute/chronic pain management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and effectively manage a mobile patient caseload.
  • Must be punctual, dependable, organized, and professional.
  • Comfortable entering and providing care in patients' homes and working with patients and caregivers in a home-based environment.
  • Valid driver's license and reliable transportation required.
  • Ability to travel throughout the assigned service area.
  • CPR/BLS certification preferred or required according to company policy.
  • Previous home health, mobile therapy, outpatient, orthopedic, neurological, geriatric, or community-based experience is preferred.
Mobile/Home-Based Care Expectations
  • Arrive at scheduled visits within the expected timeframe.
  • Manage travel time and maintain an efficient daily schedule.
  • Carry and transport necessary clinical equipment and supplies.
  • Adapt treatment approaches to different home environments and available space.
  • Maintain a safe and professional environment during every patient visit.
  • Communicate promptly regarding cancellations, scheduling changes, or patient concerns.
  • Respect patients' homes, privacy, routines, and individual preferences.
  • Demonstrate sound clinical judgment when working independently in the community.

Job Type

Full-time – 1099 Independent Contractor

Pay Structure (1099)
  • Guaranteed base pay: $1,080 per week
  • Base compensation is calculated at $45 per patient visit, based on a minimum of 6 patient visits per day.
  • $45 per additional patient visit for visits beyond the daily base of 6.
  • Compensation is subject to the terms of the independent contractor agreement.
Schedule

4/10 Schedule

  • Four 10-hour workdays per week
  • Typical start time: 7:30 AM–8:30 AM, depending on patient scheduling and geographic coverage.
  • Daily schedule may vary based on patient locations, visit duration, and patient needs.
