Job summary

FYZICAL Therapy & Balance Centers in Stamford, CT is seeking a full-time Physical Therapist to join our 1x1 patient care model. You will collaborate with staff and local healthcare professionals to deliver high-quality PT, maintain comprehensive documentation, and contribute to ongoing patient education and community outreach.

Qualifications

• Active Connecticut Physical Therapy license.
• CPR certification preferred.

Responsibilities

• Ensure smooth and timely patient flow.
• Determine necessary PT based on prescriptions/referrals/patient condition/records.
• Review patient records, strategize, create and carry out treatment plans.
• Track and report treatment progress; adjust as necessary.
• Oversee assistant, technician and student activities when necessary.
• Maintain timely documentation including daily notes, weekly reports, initial evaluations and discharge summaries.
• Plan and conduct lectures on PT and related training for in-house staff and community groups.
