Major Responsibilities:
  • Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
  • Establishes a plan of care and implements individual treatment based on evaluation results and functional goals, using appropriate techniques. Monitors outcomes and adjusts the treatment plan as needed. Provides updates to physicians and team members. Attends to factors affecting patient comfort.
  • Plans for discharge, including home programs, patient/family teaching, referrals, transition planning, and equipment ordering. Collaborates with stakeholders to ensure optimal patient care.
  • Completes documentation per department policies, including evaluations, progress notes, reports, and summaries. Manages patient care records, daily statistics, billing, and scheduling. Adheres to insurance and productivity standards.
  • Maintains a clean and organized treatment area. Performs manual techniques, patient positioning, transfers, and transport duties safely. Uses modalities and equipment knowledgeably. Promotes safety and infection control.
  • Engages in performance improvement activities. Manages caseloads and assists with coverage as needed. Pursues professional development through education and literature. Supervises assistants and students as qualified.
Minimum Job Requirements
  • Doctorate in Physical Therapy from an accredited school. Maintains continuing education as required.
  • Physical Therapy License in Illinois or Wisconsin. BLS certification.
  • No prior experience required.
  • Intermediate computer skills. Excellent communication, organization, and decision-making skills. Knowledge of growth and development principles.
Physical Requirements and Working Conditions
  • Ability to sit, stand, walk, lift, and squat throughout the day.
  • Lift up to 50 lbs from floor to waist, and up to 10 lbs overhead. Push/pull with 30 lbs of force. Perform transfers involving up to 150 lbs with assistance.
  • Repetitive hand use, including grasping, pushing, pulling, and fine manipulation. Use senses effectively for communication.
  • Exposure to hazards, requiring protective clothing. Possible travel and weather exposure. Operate necessary equipment.
