Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN positions available

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to ensure the highest quality of resident care is maintained.

Job Functions:
  1. Provide physical therapy services to residents following established standards of practice, company procedures, and productivity standards.
  2. Adhere to company procedures regarding documentation and billing of physical therapy services.
  3. Use sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
  4. Conduct regular screenings of residents to determine the need for intervention/treatment.
  5. Evaluate residents to gather data necessary for treatment planning and implementation.
  6. Develop treatment plans with short and long-term goals.
  7. Communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
  8. Complete all required documentation in compliance with CMS standards using NetHealth EMR systems.
Education & Requirements:
  • Bachelor of Science Degree in Physical Therapy from an accredited program.
  • Active Physical Therapy license in practice state(s).
  • Ongoing continuing education to maintain licensure and certification.
