Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN positions available

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as directed by the Administrator, to ensure the highest quality of resident care.

Job Functions:

• Provide physical therapy services to residents according to established standards, procedures, and productivity standards.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Evaluate residents to determine intervention/treatment needs and develop treatment plans with goals.
• Conduct screenings and specialized evaluations as needed.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Complete compliant and medically necessary documentation in NetHealth EMR systems.

Education & Requirements:

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in the state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
