Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN positions available

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as directed by the Administrator, to assure the highest quality resident care.

Job Functions:

• Provide physical therapy services to residents according to established standards, procedures, and productivity standards.
• Adhere to procedures regarding documentation and billing of services.
• Evaluate residents to determine need for intervention/treatment.
• Develop treatment plans with goals and methods.
• Communicate evaluation findings and treatment plans to residents, families, and team members.
• Complete compliant and medically necessary documentation in NetHealth EMR systems.

Education & Requirements:

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
