Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN positions available

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to assure the highest quality of resident care.

Job Functions:

1. Provide physical therapy services to residents following established standards, procedures, and productivity standards.
2. Adhere to procedures regarding documentation and billing of services.
3. Evaluate residents to develop and implement treatment plans.
4. Conduct screenings and specialized evaluations as needed.
5. Develop treatment plans with clear goals and methods.
6. Communicate findings and recommendations to residents, families, and team members.
7. Complete compliant and medically necessary documentation in NetHealth EMR systems.
8. Perform other duties as assigned to support organizational goals.

Education & Requirements:

1. Bachelor of Science Degree in Physical Therapy from an accredited program.
2. Active Physical Therapy license in the state(s) of practice.
3. Ongoing continuing education to maintain licensure and certification.
