Job summary

Legacy Healthcare Services, Inc. seeks an experienced Physical Therapist (PT) to join our team in a hybrid role combining on-site care at Oak Park Retirement with personalized home-based therapy across Rowan and Mecklenburg Counties.

You will deliver 1-on-1 sessions to senior adults, develop individualized care plans, supervise PTAs where required, and participate in interdisciplinary care while enjoying a Monday–Friday schedule and comprehensive benefits.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current Physical Therapy license in the state of North Carolina (or eligibility to obtain prior to start date).
• Excellent clinical, interpersonal, and fiscal management skills.

Responsibilities

• Comprehensive Clinical Care: Assess, plan, and deliver high-quality physical therapy services according to physician orders and the State Physical Therapy Practice Act.
• Dual-Setting Treatment: Provide individualized, 1-on-1 rehabilitation treatments on-site at Covenant Village as well as in patient homes throughout Rowan and Mecklenburg Counties.
• Environmental & Functional Assessments: Evaluate home environments, muscle strength, gait, mobility, and ROM potential to build tailored rehabilitative care plans.
• Caregiver & Family Guidance: Educate patients, families, and caregivers on home exercises, safe mobility strategies, and the proper use of therapeutic appliances.
• Interdisciplinary Collaboration: Participate in clinical conferences, coordinate integrated care plans with multidisciplinary teams, and identify referrals for additional home health (PCA) services as needed.
• Documentation & Leadership: Maintain progress summaries based on goal attainment and supervise Physical Therapist Assistants (PTAs) in compliance with state regulations and company standards.
