Legacy Healthcare Services, Inc. is seeking a compassionate Physical Therapist (PT) to join our Greensboro, NC team. This hybrid role combines on-site care at Heritage Greens with home-based therapy across Guilford County.

You will deliver 1-on-1, private sessions to seniors, develop individualized care plans, and collaborate with an interdisciplinary team while ensuring compliance with state regulations and quality standards.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current North Carolina Physical Therapy license.
• Excellent interpersonal, clinical, and fiscal management skills.
• Computer proficiency for electronic documentation and care planning.

Responsibilities

• Comprehensive Patient Care: Evaluate, assess, and provide physical therapy services per physician plans of care.
• Dual-Setting Treatment: Deliver 1-on-1 rehabilitation on-site at Heritage Greens and in patients' homes.
• Environment & Mobility Assessment: Evaluate home environments, mobility, gait, muscle strength, and ROM.
• Caregiver Education: Instruct patients, family members, and caregivers on exercises and safe mobility strategies.
• Interdisciplinary Collaboration: Coordinate care plans with a multi-disciplinary rehabilitation team.
• Documentation & Supervision: Prepare progress summaries and supervise PTAs per state regulations.

Legacy Healthcare Services is searching for an experienced, motivated, and compassionate Physical Therapist (PT) to join our team in Greensboro, NC. This unique hybrid opportunity combines on-site facility care at Heritage Greenswith personalized home-based therapy through our Better at Home program across Guilford county.

You will work directly with senior adults both in their residential community and in the comfort of their homes, empowering them to lead lives filled with purpose, safety, and joy. With private, functional-based, 1-on-1 treatment sessions, you will have the autonomy to individualize care plans and truly build meaningful relationships with your patients.

Key Responsibilities

• Comprehensive Patient Care: Evaluate, assess, and provide physical therapy services in accordance with physician plans of care and the State Physical Therapy Practice Act.

• Dual-Setting Treatment: Deliver high-quality, 1-on-1 rehabilitation services both on-site at Heritage Greens and directly in patients' homes throughout Greensboro/Guilford County.

• Environment & Mobility Assessment: Evaluate home environments, mobility, gait, muscle strength, and ROM potential to build tailored rehabilitation programs.

• Caregiver Education: Instruct patients, family members, and caregivers on therapeutic appliances, home exercises, and safe mobility strategies.

• Interdisciplinary Collaboration: Participate in clinical conferences and coordinate care plans as part of a multi-disciplinary rehabilitation team. Identify additional patient needs (such as PCA services) and make appropriate referrals.

• Documentation & Supervision: Prepare timely progress summaries and supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.

Pay Range

USD $95,000.00 - USD $110,000.00 /Yr.
