Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity providing home health visits in our senior living office. The office services adult and geriatric clients in Western Montgomery County.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities include:

  1. Making home visits to clients within designated territories.
  2. Performing diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, endurance, balance, and safety assessments.
  3. Implementing wound prevention and repair interventions using evidence-based dressings and topical agents.
  4. Developing and updating individualized care plans, including manual exercises, gait training, and balance interventions.
  5. Assessing and revising care plans regularly and collaborating with nursing staff on multidisciplinary care.
  6. Educating clients and their families on rehabilitative activities to promote health, safety, and independence.
  7. Documenting observations, interventions, and evaluations accurately using a tablet device.

Qualifications:

  • Current PA Physical Therapist (PT) license.
  • Graduation from an American Physical Therapy Association-approved program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Proficiency with computers; experience with EMR preferred.
