Physical Therapist (PT) - Home Health Senior Living

BAYADA Home Health Care is seeking a Physical Therapist (PT) for a per diem home health position in our senior living office located in Raleigh, North Carolina. The office serves adult and geriatric clients across territories in Wake, Durham, and Chapel Hill.

Requirements: Must be available 2-3 days/week and willing to travel within the specified areas.

Role Overview

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities
  • Make home visits to clients in designated territories.
  • Perform diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, endurance, balance, and safety assessments.
  • Implement wound prevention and repair interventions using evidence-based dressings and topical agents.
  • Develop and execute individualized care plans, including manual exercises, gait training, and balance interventions.
  • Assess and update the care plan regularly, collaborating with nursing staff.
  • Educate clients and families on rehabilitative activities to promote health and independence.
  • Document all client interactions and interventions accurately using electronic tablets.
Qualifications
  • Current North Carolina Physical Therapist license.
  • Graduation from an APTA-approved program or equivalent.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Proficiency in computer skills; experience with EMR preferred.
