Key Responsibilities

• Promote Elara Caring’s philosophy, mission, and policies to ensure quality care
• Provide direct patient care, evaluate needs and outcomes, and consult with specialists. Adjust care plans for optimal results.
• Design and implement patient care plans based on assessments, involving significant others when appropriate.
• Conduct therapy within standards of care.
• Lead support staff to achieve patient goals.
• Document evaluations, treatment goals, plans, and maintain updated records.
• Provide leadership and act as a resource to promote physical therapy in the community.
• Work in a physically demanding, high-stress environment.
• Handle, lift, and transfer patients safely.
• Potentially work irregular hours, including call hours.

Minimum Requirements

• Graduate from an APTA-approved physical therapy program
• Current, unrestricted PT license for the state of work
• At least one year of experience in a healthcare facility
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver’s license, and auto insurance
• Willing to travel up to 50%
• Ability to sit, stand, bend, lift, and move intermittently; able to lift 50-100 lbs

Compensation starts at $75 per point, with potential bonuses for performance, work beyond quota, and on-call hours. Compensation varies based on education, experience, and location.

You will report to the Branch Director, Clinical Manager, or designee.
