Key Responsibilities

• Promote Elara Caring’s philosophy and policies to ensure quality care
• Provide direct patient care, evaluate needs, and adjust care plans for optimal outcomes
• Design and implement individualized care plans
• Conduct therapy within standards of care
• Lead support staff to achieve patient goals
• Document evaluations, treatment goals, and plans accurately
• Provide leadership and promote physical therapy in the community
• Work in a physically demanding, high-stress environment
• Handle, lift, and transfer patients safely
• Potentially work irregular hours including call hours

Minimum Requirements

• Graduate from an APTA-approved physical therapy program
• Current, unrestricted PT license for the state of work
• At least one year of healthcare facility experience
• Willing to travel within the coverage area
• Dependable vehicle, valid driver’s license, auto insurance
• Willing to travel 50%
• Ability to sit, stand, bend, lift, and move intermittently, lifting 50-100 lbs

You will report to the Branch Director, Clinical Manager, or designee.
