Job description

What you will do:
  • Conduct initial and ongoing assessments of patients, including OASIS assessments at appropriate times
  • Perform assessments such as muscle examinations, gait analysis, sensory testing, and range of motion measurements
  • Develop and evaluate care plans in collaboration with patients, caregivers, physicians, and the care team
  • Provide services ordered by the physician as per the care plan
  • Administer therapeutic treatments and assess equipment needs
  • Communicate with physicians and healthcare practitioners regarding patient progress and condition changes
  • Instruct patients on the use of equipment like wheelchairs, braces, and prosthetics
  • Evaluate the home environment and make recommendations
  • Consult and instruct family and agency personnel involved in the patient's therapy
  • Provide education and counseling tailored to individual needs
Who you are:
  • Qualified with education and credentials as per § 484.115(h) (Physical Therapist)
  • Licensed as a physical therapist by the Arizona State Board of Physical Therapy
  • Possess core competencies per skills checklist
  • At least one year of physical therapy experience
  • Clear background check including fingerprint card, DMV, and criminal record
  • Current CPR, TB, and health clearance
  • Complete 6 hours of in-service training annually
