What you will do:
- Conduct initial and ongoing assessments of patients, including OASIS assessments at appropriate times
- Perform assessments such as muscle examinations, gait analysis, sensory testing, and range of motion measurements
- Develop and evaluate care plans in collaboration with patients, caregivers, physicians, and the care team
- Provide services ordered by the physician as per the care plan
- Administer therapeutic treatments and assess equipment needs
- Communicate with physicians and healthcare practitioners regarding patient progress and condition changes
- Instruct patients on the use of equipment like wheelchairs, braces, and prosthetics
- Evaluate the home environment and make recommendations
- Consult and instruct family and agency personnel involved in the patient's therapy
- Provide education and counseling tailored to individual needs
Who you are:
- Qualified with education and credentials as per § 484.115(h) (Physical Therapist)
- Licensed as a physical therapist by the Arizona State Board of Physical Therapy
- Possess core competencies per skills checklist
- At least one year of physical therapy experience
- Clear background check including fingerprint card, DMV, and criminal record
- Current CPR, TB, and health clearance
- Complete 6 hours of in-service training annually
