What You'll Do :

• Conduct comprehensive assessments of patients' needs in their home environment
• Develop and implement individualized treatment plans to enhance patients' ability to perform daily functions
• Provide skilled physical therapy services, adjusting interventions as necessary to achieve patient goals
• Educate patients and caregivers on strategies to improve functional mobility and independence
• Ensure timely and accurate documentation in compliance with OASIS and regulatory standards
• Collaborate with a multidisciplinary team to deliver optimal care
• Maintain positive, professional relationships with patients, families, and colleagues

Scheduling Options :

• Full-Time : Consistent caseload with flexible daily scheduling
• PRN : Flexible, as-needed visit assignments based on your availability. No set shifts. You manage your schedule

Compensation :

• Pay-per-unit model for both Full-Time and PRN roles
• Competitive rates based on visit type and experience

Requirements

What You'll Need :

• Active Physical Therapist license in Arizona
• 1-2 years of Home Health experience required
• Fluency in OASIS required (KanTime knowledge a plus)
• Arizona Level One Fingerprint Clearance Card required
• Valid Driver's License and car insurance card
• Current CPR and First Aid certification required
• Current TB test
• Two professional references required at time of interview
• Compassionate, patient-focused approach to care
