ABL Health Care in Hayward, CA is seeking a dedicated Physical Therapist (PT) to join our Home Health team full-time. You will evaluate patients, develop care plans, and monitor progress in the patient’s home setting, collaborating with our Clinical Director and staff.

The role emphasizes compassionate, evidence-based care, with support from a back office and access to tools that streamline documentation. A valid California PT license and at least one year of home health experience are required,

Qualifications

• Current California PT license to practice.
• At least 1 year of home health or hospice care experience.
• Current California driver’s license.
• Ability to travel within a 15–25 mile radius on a regular basis.

Responsabilités

• Provide, coordinate, and oversee skilled intermittent care to patients in their homes and instruct patients and caregivers to promote maximal functional status.
• Regularly evaluate and re‑evaluate patient status.
• Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, and Director of Nursing as needed.
• Initiate, develop, implement, and revise the patient’s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs related to the patient’s condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.

We are currently seeking a compassionate, organized, and team-oriented individual to join our Home Health team as a Full‑Time Physical Therapist (PT) in Hayward.

Position Overview

As a Physical Therapist (PT) you will evaluate patients’ health, create and implement care plans, and track their progress to maintain the highest standards of care. You will work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.
