HomeFirst Home Healthcare is seeking a full-time Physical Therapist to join our home health clinical field team in Bibb County, Alabama, and surrounding areas. You will work directly with patients in their homes, conduct assessments, develop personalized treatment plans and implement therapeutic interventions to improve functional abilities.

Formación

• Graduate of an accredited physical therapy educational program.
• Current PT license in Alabama.
• Current CPR certification.
• Excellent organizational, time-management, written and verbal communication skills.
• Reliable transportation, valid driver’s license and current automobile insurance.

Responsabilidades

• Provide one-on-one patient care and patient/family teaching in the home setting.
• Assess patient needs, develop personalized treatment plans and implement therapeutic interventions.
• Help patients optimize functional abilities through therapy.
• Collaborate with family and care team to ensure continuity of care.

Overview

As a home health Physical Therapist, you would be responsible for conducting one-on-one patient care and patient/family teaching in the home setting. Requirements
