Five Star Home Health Care LLC in Chantilly, VA is seeking a Physical Therapist (PT) for a full-time, in-home care role. In-person, Monday–Friday, you will render 1-on-1 treatment in the comfort of the patient’s home with a company car you can take home and a gas card.

We use paperless documentation with Homecare Homebase EMR and follow Care Maps to guide treatment. Expect 30/60/90 day feedback, 2 weeks PTO, 8 paid holidays, 401k with up to 3% company match, and tuition reimbursement up to $3400

Qualifications

• Bachelor’s degree in physical therapy or related field.
• One (1) year working in a home health setting is required.
