BAYADA Home Health Care is currently seeking a Physical Therapist (PT) for a full-time opportunity providing home health visits, assessments, and admissions. This role services adult and geriatric clients in the territories of Stokes and Surry counties.

As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team, offering rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include:

• Making home visits to clients in designated areas.
• Performing diagnostic tests such as mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Implementing wound prevention and repair interventions using evidence-based dressings and topical agents.
• Developing and executing individualized care plans, including manual exercises, gait training, and balance interventions.
• Assessing and updating care plans regularly, collaborating with nursing staff as needed.
• Educating clients and families on rehabilitative activities to promote health, safety, and independence.
• Documenting observations and interventions accurately using a state-of-the-art tablet.

Qualifications include:

• Current North Carolina Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers and electronic medical records (EMR).
