Responsibilities

• Provide home-based physical therapy to patients with mobility, strength, and functionality issues
• Collaborate with a home health team including RN, LPN, CNA, HHA, OT, and SLP
• Observe, assess, document, and modify care plans
• Assist with exercises and hands-on therapy
• Educate patients and families about care plans and exercises

Minimum Qualifications

• Graduate of an accredited Physical Therapist Program and active PT license in Wisconsin
• Minimum of 1 year of physical therapy experience, preferably in home healthcare
• CPR certification
• Knowledge of home health regulations
• Strong clinical judgment, interpersonal skills, and compassion
