HealthFlex Home Health & Hospice in California is seeking a licensed Physical Therapist to provide in-home care. You will assess mobility, design treatment plans, and deliver therapies to improve function at patients’ homes.

The role offers flexible scheduling, supportive staff, and opportunities for professional growth within a large privately owned agency serving the Bay Area, Central Valley, and Sacramento region.

Qualifications

• Current and unrestricted CA PT License.
• 1 year PT experience.
• Current AHA CPR certification.
• Valid Driver's License.

Responsibilities

Position Summary

Home Health Physical Therapists (PT) provide personalized care to patients in their homes, focusing on improving mobility, managing pain, and enhancing overall physical function. They assess patients' needs, design tailored treatment plans, and conduct therapies to promote recovery and well-being within the comfort of home settings.

Compensation

Salary Description
