Job summary

Mission Healthcare is hiring a Physical Therapist to evaluate patients, design individualized treatment plans, and implement therapeutic exercises in a home health setting in Maricopa, AZ. You will educate patients and caregivers, collaborate with a multidisciplinary team, and document progress accurately.

Requirements include an active PT license, Master's or Doctorate in Physical Therapy, 1 year of PT experience (home health preferred), BLS/CPR, and a valid driver's license with insured

Qualifications

• Active Physical Therapist license in practicing state.
• Master's or Doctorate degree in Physical Therapy.
• Minimum of 1 year of experience as a Physical Therapist, home health experience strongly preferred.
• Current BLS, hands-on CPR certification.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate and assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.

Skills

SOC: $140-$150

Recert: $120-$130

Revisit: $90

Eval: $120-$130

Discharge Oasis: $95.00

Discharge: $90.00

Hourly: $35-$40, depending on experience
