Job description

Job Opportunity: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem home health position in our Person County office, located in Roxboro, North Carolina. This role involves providing home visits to adult and geriatric clients across Franklin/Nash county areas.

As a home care Physical Therapist, you will be an integral part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities

1. Make home visits to clients in designated territories.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength, endurance, balance, and safety assessments.
3. Develop and implement individualized care plans with manual therapeutic exercises, gait training, and other interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic medical records.
7. Implement wound care interventions using evidence-based dressings and topical agents.

Qualifications

• Current North Carolina Physical Therapist (PT) license.
• Graduation from an accredited program recognized by the American Physical Therapy Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; prior EMR experience preferred.
