Mission Healthcare is seeking a full-time Physical Therapist to provide home health services across our patient population. You will evaluate patients, design personalized treatment plans, and guide exercises to improve mobility and reduce pain in a home setting.

You will collaborate with an interdisciplinary care team, educate patients and caregivers, and maintain precise progress documentation to support ongoing care quality and outcomes.

Qualifications

• Active Physical Therapist license in practicing state.
• Master's or Doctorate degree in Physical Therapy.
• Minimum of 1 year of experience as a Physical Therapist, home health experience strongly preferred.
• Current BLS, hands-on CPR certification.
• Valid driver's license and reliable, insured vehicle.

• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.

• Maintain accurate and timely documentation of patient progress and treatment plans.

Pay range: Starting at $60/hour , depending on experience
