Job summary

Mission Healthcare in South Orange County, CA is seeking a licensed Physical Therapist for per diem home health and hospice care. You will assess patients, develop individualized treatment plans, and implement therapeutic exercises to improve mobility and reduce pain while collaborating with an interdisciplinary team.

You will educate patients and caregivers on exercises, safety, and self-care, maintain timely documentation, and participate in a patient-centered care approach.

Qualifications

• Active Physical Therapist license in practicing state.
• Master's or Doctorate degree in Physical Therapy.
• Minimum of 1 year of experience as a Physical Therapist; home health experience preferred.
• Current BLS, hands-on CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Evaluate and assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.

Pay range: $65-68 depending on experience
