Sequoia Home Health is seeking a Physical Therapist to provide in-home therapy services. The PTA will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence in their own homes.
The role requires an active California Physical Therapy License, at least 2 years of PT experience (home health preferred), strong communication skills, current CPR certification, and a valid driver’s license with reliable vehicle.

Pay Range: $130,000 - $200,000 annually

We are seeking a Physical Therapist to provide in-home therapy services. The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence.
Evaluate patients’ functional status and develop care plans
Assess home environments and recommend modifications
Provide direct physical therapy treatments
Educate patients and caregivers on therapeutic techniques and equipment
Monitor progress and adjust treatment plans
Collaborate with physicians and interdisciplinary teams
Document clinical notes and progress reports
Participate in discharge planning
Supervise Physical Therapy Assistants per policy and regulations
Active California Physical Therapy License
Minimum 2 years of physical therapy experience (home health preferred)
Strong communication and organizational skills
Current CPR certification
Valid driver’s license and insured, reliable vehicle
