Eligible employees also receive paid time off, access to a 401(k), company-paid life insurance, and an Employee Assistance Program.HOW YOU'LL WORK:You will deliver skilled physical therapy services in accordance with physician orders, ensuring patient safety while providing education to both patients and their family members. You will be responsible for maintaining accurate and compliant clinical documentation in alignment with all applicable regulations and standards.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Provide skilled physical therapy as ordered by the attending physician. Observe, document, and report patient responses, progress, and any changes in condition. Conduct OASIS assessments to establish and adjust individualized plans of care as needed.Patient Outcomes: Deliver interventions to relieve pain, restore or improve function, and maintain optimal performance using therapeutic techniques such as exercise, manual therapy, and modalities.Minimize Patient Risk: Reduce re-hospitalizations by prioritizing visits for high-risk patients, providing education on preventive measures, and promoting informed decision-making.Family Educator/Advocate: Educate patients and families on disease or injury management throughout all phases of therapy. Provide instruction on the use of mobility aids and adaptive equipment, including wheelchairs, braces, walkers, crutches, canes, and prosthetic or orthotic devices.Supervision: Supervise and oversee therapy services provided by Physical Therapist Assistants (PTAs) and, when appropriate, nurse aides.Communication/Collaboration: Maintain consistent communication with PTAs and nurse aides regarding supervisory visit findings and care coordination.Policies: Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.Rules and Regulations: Adhere to all agency policies, procedures, and regulatory requirements.
