Job Summary

Provide physical therapy services to patients in their residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range from birth to over 100 years old. Collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate services.

Key Activities

1. Develop patient- and family-centered care plans with standardized interventions for goal achievement.
2. Ensure daily documentation is accurate and timely.
3. Apply standard treatment procedures.
4. Provide teaching to patients and caregivers.

Qualifications and Experience

• At least one year of experience in acute care, rehab, or homecare as a Physical Therapist.
• Registered with the Massachusetts Board of Registration.
• Excellent communication skills, both verbal and written.
• Proficiency in computer use and ability to learn new software.
• Ability to prioritize and adapt to changing situations.
• Travel within the geographic area of Mass General Brigham Healthcare at Home is required.
