Avista Nursing and Rehab in Saginaw, Michigan is seeking a dedicated Physical Therapist (PT) to join our patient-centered care team. You will evaluate patients, develop individualized treatment plans, and deliver therapy to improve strength, flexibility, and mobility.

You will collaborate with doctors and nurses, educate patients and families on transfers and safety, maintain accurate records, and stay current with best practices in physical therapy to improve patient outcomes.

Qualifications

• Degree in Physical Therapy (PT) from an accredited institution; current licensure or certification as a Physical Therapist.

Responsibilities

• Conduct thorough evaluations and assessments of patients' physical abilities and develop individualized treatment plans to achieve rehabilitation goals.
• Develop and implement physical therapy interventions to improve strength, flexibility, and range of motion.
• Collaborate with other healthcare professionals to develop comprehensive care plans for each patient.
• Provide education to patients, families, and caregivers on transfer techniques, ambulation, and safety strategies.
• Maintain accurate records of patient progress and care delivery per regulatory requirements and facility policies.
• Participate in interdisciplinary team conferences to contribute to care planning.
• Stay up-to-date with current best practices in physical therapy to improve outcomes.
