• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in outpatient settings.
• Design and implement personalized treatment plans to restore function and improve quality of life.
• Accurately document patient progress using EMR systems.
• Collaborate with multidisciplinary teams to deliver holistic, patient-centered care.
• Maintain a positive, professional clinic environment reflecting H2 Health’s standards.

Requirements:

• Degree from an accredited Physical Therapy program (DPT or equivalent).
• Active or eligible state license to practice as a PT.
• New graduates are welcome! Outpatient experience is a plus but not mandatory.
