The Grove at Valhalla Rehabilitation and Nursing Center is seeking a licensed Physical Therapist (Weekend PRN) to join our rehabilitation team. Our 160-bed facility offers a state-of-the-art Rehabilitation Gym and serves 50+ short-term beds within a comprehensive therapy program.

New graduates are welcome. Responsibilities include patient assessment, developing and administering treatment plans, documentation, and family education. The center maintains seven-day rehabilitation services.

Formación

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid NY license and registration as a Physical Therapist.
• Experience with long-term care/geriatric care preferred.
• Strong documentation skills: evaluations, progress notes, recertification, discharge.
• Collaborates with all disciplines to provide patient-centered care.
• Punctual, professional, and compliant with confidentiality/HIPAA.

Responsabilidades

• Assess referred patients and provide direct patient care to determine rehabilitation potential.
• Establish and administer a treatment plan with goals based on patient capacity and tolerance.
• Supervise Physical Therapy Assistants and ensure quality patient care.
• Document patient status and educate patients and families in therapy methods.

The Grove at Valhalla Rehabilitation and Nursing Center is looking for a talented and hard-working Licensed Physical Therapist (Weekend PRN) to join our ever-growing rehabilitation team.

The Grove at Valhalla Rehabilitation and Nursing Center is a facility with 160 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 50 + short term beds. The Grove at Valhalla Rehabilitation and Nursing Center team consists of 20+ Therapists spanning 20 years of experience with backgrounds in acute, sub-acute, long term and outpatient settings which provides an excellent setting for learning and growth. The Grove at Valhalla Rehabilitation and Nursing Center provides Rehabilitation services 7 days a week.

Responsibilities Include:

The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Physical Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate physical therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of NY
• The Physical Therapist is responsible for overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
