Job summary

Ahava Therapy Services is seeking a PRN Physical Therapist (PT) to join the Maplewood Healthcare in Jackson, TN. The role supports a rehab team providing therapy across short- and long-term care with outpatient services.

You will evaluate residents, develop individualized plans, supervise assistants, and document progress while collaborating with nurses, families, and other professionals to maximize function and quality of life.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Current Tennessee PT license.
• Current BLS/CPR certification.
• Strong observational, verbal, and written communication skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Review resident information and evaluate impact on assessment and treatment.
• Conduct functional resident assessments using standardized procedures.
• Develop and implement individualized treatment plans with functional goals.
• Comply with evaluation, treatment, and documentation guidelines; maintain records.
• Instruct assistants in treatment procedures and provide supervision.
• Enter treatment and progress notes after each session.
• Revise treatment plans as needed based on resident performance.
• Document discharge summaries clearly and concisely.
• Orient residents to program services and care appropriate to their needs.
• Collaborate with nursing staff, residents, and families to maximize function.
• Educate residents and caregivers on home exercises and procedures.
• Assist with referrals post-discharge; participate in team conferences.
• Access continuing education to maintain clinical competency and quality.

Responsibilities

• Review available resident information and evaluate the impact on assessment and treatment.

• Conduct and document thorough functional resident assessments using a standardized procedure.s

• Comply with evaluation, treatment, and documentation guidelines; maintain timely and accurate records.

• Instruct Assistants in treatment procedures and provide supervision while they work with residents

• Assist with referral processes for additional services post-discharge

• Attend and participate in rehabilitation team conferences and other facility meetings

• Access continuing education opportunities to maintain clinical competency

• Participate in quality improvement activities

• Adhere to Ahava Therapy Services’ regulatory and professional standards

• Follow facility policies and participate in safety programs

• Perform other duties as assigned
