This position is for an Independent Contractor serving the Lake area specifically.

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver exceptional care and work with adults of all ages.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient needs to promote movement, reduce pain, restore function, and prevent disability. Based on this assessment, the therapist develops a treatment plan, executes interventions to improve patient well-being, and monitors progress effectively.

Requirements for a Physical Therapist in Home Health:

• Strong knowledge of physical therapy principles, theories, and methods applied in rehabilitation
• Skills in providing various treatments
• Effective written and oral communication abilities
• Understanding of therapy's role considering the patient's age, needs, and environment
• Accuracy and discretion in record-keeping
• Sound clinical reasoning in treatment planning and patient progress monitoring
• Ability to recognize when treatment plans need modification and report accordingly

Qualifications:

• Enthusiasm and ability to work with all ages
• State licensure and registration as a Physical Therapist
• Completion of an accredited Physical Therapist curriculum approved by the relevant authorities
