Job description

This position is for an Independent Contractor to serve the Yountville area.

FeldCare Connects

is currently seeking a self-motivated Physical Therapist to deliver excellent care and work with adults of all ages.

The Physical Therapist in Home Health is responsible for assessing and evaluating patient needs to promote movement, reduce pain, restore function, and prevent disability. Based on this assessment, the therapist develops a treatment plan, implements interventions to improve the patient's well-being, and monitors progress.

Requirements for a Physical Therapist in Home Health

• Extensive knowledge of physical therapy principles, theories, and methods applicable in a rehabilitation setting.
• Skills in providing various treatments.
• Strong communication skills, both written and oral.
• Ability to understand the role of therapy considering the patient's age, needs, and environment.
• Ability to accurately prepare and maintain records with discretion.
• Sound clinical reasoning in treatment planning, implementation, and monitoring patient progress.
• Ability to recognize when treatment plans need modification and report accordingly.

Qualifications

• Enthusiasm and ability to work with all ages.
• State licensure and registration as a Physical Therapist.
• Completion of an APTA-approved Physical Therapist curriculum.
• Bilingual skills are a plus.
