Overview

Evaluates and treats patients of all ages, modifying plans of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students.

Responsibilities
Essential Functions
  1. Demonstrates ability to perform patient transfers with assistance as needed from bed, chair, and floor, if applicable. Uses safe and appropriate body mechanics to reduce injury risk to self, staff, and patients.
  2. Shows respect by treating patients and colleagues with dignity and empathy, staying fully engaged with those receiving services.
  3. Designs individualized care plans that include goals, treatments, outcomes, and discharge plans. Adjusts care based on patient response and evidence.
  4. Educates patients, families, caregivers, students, and healthcare providers using effective teaching methods.
  5. Evaluates clinical findings to determine physical therapy diagnoses and care outcomes.
  6. Performs examinations using reliable and valid methods relevant to the chief complaint, screening results, and history.
  7. Produces documentation in accordance with the Clinical Process Policy (CTSCP67).
  8. Screens patients to assess the effectiveness and need for therapy services, if appropriate.
  9. Uses support personnel ethically and legally, monitors their care, and provides feedback.
  10. Completes annual education as required to comply with DNV Hip and Knee Replacement Program requirements, if applicable.
Department Specific Job Functions
  1. Evaluates impairments related to orthopedic and neurologic conditions.
  2. Establishes care plans based on functional deficits and patient goals.
  3. Performs objective testing for range of motion, strength, and functional performance.
  4. Documents daily in therapy EMR and charges accordingly.
  5. Applies tape for stabilization, facilitation, or inhibition of movement.
  6. Performs therapeutic modalities such as ultrasound, interferential, and iontophoresis.
  7. Communicates progress and treatment recommendations with referring providers.
Qualifications
Education
  • Bachelor's Degree in Physical Therapy
Licensure/Certification
  • Licensed Physical Therapist (PT) in Illinois (IDFPR)
  • Basic Life Support (BLS) certification within 30 days (AHA)
  • Completion of required continuing education for acute stroke care

Salary Range
