Responsibilities
  • Interpret physician referrals and complete evaluations (musculoskeletal, neuromuscular, cardiopulmonary, integumentary) using appropriate methods.
  • Establish and implement a personalized plan of care based on evaluation results and functional goals, utilizing suitable techniques.
  • Monitor and evaluate patient outcomes, adjusting treatment plans as needed, and communicate progress to physicians and team members.
  • Address factors affecting patient's physical and psychosocial comfort.
  • Plan for and execute patient discharge procedures, including home programs, education, referrals, and equipment orders.
  • Complete all documentation in accordance with policies, including evaluations, progress notes, and discharge summaries.
  • Maintain a clean and safe treatment environment, using proper techniques for manual therapies and equipment.
  • Participate in performance improvement activities and manage caseloads effectively.
  • Pursue ongoing professional development and assist in training new staff or supervising assistants and students as applicable.
Minimum Job Requirements
  • Education: Doctorate in Physical Therapy from an accredited institution; ongoing continuing education.
  • Certification/License: Valid Physical Therapy License in Illinois or Wisconsin; BLS certification from AHA.
  • Work Experience: No prior experience required.
Skills and Abilities
  • Intermediate computer skills.

Physical Requirements
  • Ability to sit, stand, walk, lift (up to 50 lbs.), and perform manual techniques safely.
