Summary

The Physical Therapist (“PT”) evaluates and assesses patients' needs and formulates treatment plans in the home care environment. Responsibilities include providing therapy services, creating and overseeing care plans, and supervising physical therapy assistants. The PT collaborates with the interdisciplinary care team, physicians, and other stakeholders.

Primary Responsibilities

• Determine treatment plans through assessments and consultations.
• Complete all documentation accurately.
• Administer treatments as ordered and oversee assistants.
• Evaluate treatment effects and recommend adjustments.
• Complete discharge planning and ensure continuation of care post-discharge.
• Participate in weekly case conferences and communicate effectively across disciplines.

Qualifications

• Minimum 1 year of physical therapy experience, preferably in acute or post-acute care.
• Active state Physical Therapist licensure.
• Valid CPR certification.
• Valid Driver’s License.
