Physical Therapist (PT)

*Full Time*

SSM Health Physical Therapy is seeking a motivated full-time physical therapist with an interest in building an outpatient caseload of patients who prefer or need to be seen in their homes. This position will start as a hybrid role, with hours not spent in patients' homes allocated to working in a float capacity within our outpatient centers in the mid-county market (40/270 corridor).

Our At-Home patients will be evaluated and treated in their homes in a safe and effective manner. The therapist will be provided with all necessary equipment to treat patients in a home setting and will document appropriately during sessions. If needed, the therapist will transition patients to a local center for additional or specialized care.

Responsibilities

• Evaluate assigned patients and develop, implement, and modify treatment plans based on patient needs and physician directives
• Delegate and oversee therapy services for assigned patients
• Design functional treatment programs to facilitate effective community reintegration
• Educate patients, families, students, and coworkers
• Perform other related professional duties, including developing therapy services and program delivery

Qualifications

• Bachelor’s Degree in Physical Therapy required
• Current and valid state licensure or eligibility required
• CPR certification required
