Elara Caring is seeking a Physical Therapist to deliver high-quality home health care. The role involves patient evaluations, treatment planning, and interdisciplinary collaboration to promote independence and mobility in patients’ homes.

Primary duties include designing personalized care plans, documenting progress, and serving as a clinical resource in the community. Willingness to travel up to 50% and lift up to 50 pounds is required.

Competenze

• Graduate of an APTA-accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license in the state of practice.
• Minimum of one year of experience in a healthcare setting.
• Willingness and ability to travel up to 50% within the assigned coverage area.
• Dependable vehicle, valid driver’s license, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds.

What You’ll Do

• Work in a physically demanding, fast-paced home-care environment

• Perform lifting, transferring, and positioning of patients as required

• May work irregular hours or participate in call rotation, if applicable

What We’re Looking For

• Graduate of an APTA-accredited Physical Therapy program

• Current, unrestricted Physical Therapist license in the state of practice

• Minimum of one (1) year of experience in a healthcare setting

• Willingness and ability to travel up to 50% within the assigned coverage area

• Dependable vehicle, valid driver’s license, and current auto insurance

• Ability to lift, push, pull, and support up to 50 pounds

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee .
