Job summary

Elara Caring is seeking a Physical Therapist to deliver high-quality home-based care across our patient population. You’ll evaluate, treat, and design individualized plans to help patients regain mobility and independence, while collaborating with a multidisciplinary team in a fast-paced home-care setting.

The role offers a $5,000 sign-on bonus, competitive compensation, tuition reimbursement for full-time staff, and ongoing continuing education opportunities.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program.
• Current, unrestricted Physical Therapist license in the state of practice.
• Minimum of one year of experience in a healthcare setting.
• Willingness and ability to travel up to 50% within the assigned coverage area.
• Dependable vehicle, valid driver’s license, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds.

Responsibilities

• Promote Elara Caring’s mission, philosophy, and policies to ensure exceptional patient care.
• Provide direct patient care, including evaluations, treatment planning, and outcome assessments.
• Design and implement individualized plans of care tailored to each patient’s needs.
• Collaborate with interdisciplinary team members to ensure optimal outcomes.
• Direct and support therapy assistants and staff to achieve patient goals.
• Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders.
• Serve as a clinical resource and promote physical therapy within the community.
• Work in a physically demanding, fast-paced home-care environment.
• Perform lifting, transferring, and positioning of patients as required.
• May work irregular hours or participate in call rotation, if applicable.

What We’re Looking For
