About the Role

Make an impact through our Home and Community Program! Starting compensation is at least $80,000 annually, with a $10,000 Sign-on Bonus available. We offer an optional dual role of Clinical Coordinator with partial remote work.

Responsibilities

• Perform evaluations and develop treatment plans
• Reassess treatment effectiveness regularly
• Conduct discharge evaluations and provide safety recommendations
• Communicate progress to the team, physicians, clients, and families
• Collaborate with other disciplines to enhance patient outcomes

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program
• Current, unrestricted PT license in practicing state
• Basic Life Support or CPR certification as per state regulations
• At least one year of relevant experience preferred
• Knowledge of neurological injury rehabilitation techniques
• Effective communication skills
• Ability to lift up to 50 pounds, with physical capability for patient transfers up to 100 pounds
• Physical requirements include pushing, pulling, bending, reaching, kneeling, and standing for extended periods
