HealthPro Heritage seeks a compassionate Physical Therapist for an inpatient SNF at Maple Lane Nursing Home in Barton, VT. This full-time role offers a diverse caseload and a supportive interdisciplinary team dedicated to patient outcomes and professional growth.

You will assess mobility, strength, and function; implement evidence-based treatments; educate patients and families; and maintain accurate documentation.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid state licensure as a Physical Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills; proficient in developing and implementing effective treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.
• Ability to lift and assist patients; stand for extended periods.

Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, mobility, strength, and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy, and educate patients and families.
• Documentation: Maintain accurate records of evaluations, progress, and plans.
