Job summary

InHome Therapy is seeking a licensed physical therapist to provide in-home care to adults and seniors in the St. Charles area of Illinois. You will assess mobility, develop treatment plans, and implement exercises in patients’ homes, ensuring safety and progress.

The role requires a valid PT license, CPR, reliable transportation, and comfort with EMR systems; mileage reimbursement and flexible scheduling are offered to support patient-focused care.

Qualifications

• Active PT license in good standing.
• BLS CPR certification is required.
• Reliable transportation to see patients in their homes.
• Familiarity with email, texting, and EMR systems (training provided).

Responsibilities

• Deliver in-home physical therapy services to adults and geriatric patients.
• Evaluate mobility, strength, and function to set goals.
• Implement rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate home health care documentation per Medicare and agency standards.

• The evaluation visit rate for this role ranges from $65 - $85 per visit

Key Responsibilities

Requirements

• Basic comfort with technology - email, texting, EMR systems (training provided)

• This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
