Synchrony Rehab is seeking a licensed Full-Time Physical Therapist to join our dynamic rehab team at The Landings of Smithfield, a senior living community located in Smithfield, NC.

(Ideal candidate would provide in-person services at The Landings of Smithfield and telehealth services to other senior living communities in NC)

Qualifications

Job Summary: The Physical Therapist (PT) provides a full range of services including assessment, treatment planning, and interventions in an interdisciplinary environment, adhering to professional and ethical standards.

Licenses and Certifications: Must hold a current, valid state PT license or certification. Current CPR certification preferred.

Education: Degree in Physical Therapy from an accredited program.

Experience: 0-1 years.

Roles and Responsibilities

• Conduct assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Educate patients and families on exercises, posture, and strategies to manage conditions.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Collaborate with the interdisciplinary team for comprehensive patient care.
• Perform other duties as assigned.
