Job summary

Synchrony Rehab is seeking a Physical Therapist to provide comprehensive PT services in Seymour, Indiana. You will assess patients, plan treatments, and implement therapeutic interventions to improve function and quality of life.

Qualifications

• Must hold a current, valid state PT licensure or certification.
• CPR certification is preferred.
• Degree in Physical Therapy from an accredited program.

Responsibilities

• Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy, and modalities to improve function.
• Educate patients and families on exercise techniques, posture, body mechanics and self-management.
• Maintain accurate documentation of progress and treatment plans per facility policies and regulatory requirements.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.
• Perform other duties as assigned.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

Licenses and Certifications

Must have and maintain a current, valid state PT licensure or certification
