Job summary

Sweetwater Care in Seigler Springs, CA seeks a licensed Physical Therapist to provide comprehensive patient care across the treatment continuum. You will develop individualized plans, perform assessments, and document progress in an electronic system.

You will supervise PT assistants and aides, coordinate with doctors and families, and ensure discharge planning meets regulatory standards. A California PT license is required.

Qualifications

• Active State Licensure as a Physical Therapist.
• Ability to supervise and support physical therapist assistants, rehabilitation aides and/or students.

Responsibilities

• Consult with patients to learn about their physical condition and review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan.
• Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing.
• Maintain patient record by keeping track of goals.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Modify services in relation to patient evaluation and referrals.
• Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.
• Communicate with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records per company policy.
• Participate in facility and company trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to company policies and code of conduct.
