Job summary

  • Current and unrestricted California Physical Therapist (PT) license required.
  • Strong clinical judgment and documentation skills.

  • Ability to motivate and engage residents throughout the rehabilitation process.
  • Positive, team-oriented attitude with a passion for helping others regain independence.

Responsibilities

  • Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals and clinical needs.
  • Provide skilled, evidence-based physical therapy treatments in accordance with the California State Practice Act.
  • Supervise PTAs and support Restorative Nursing Aides to ensure continuity of care.
  • Maintain timely documentation, including daily treatment notes, progress reports, and recertifications.
  • Collaborate with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as needed.
  • Educate residents and families on mobility safety, home exercise programs, and discharge planning.
  • Coordinate durable medical equipment such as walkers, wheelchairs, and braces to support recovery.

Key Responsibilities

As a Physical Therapist at Canyon Springs Post Acute, you will play a vital role in helping residents regain strength, mobility, and independence.
