Job summary

Canyon Springs Post Acute in San Jose, CA is seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. This full-time role offers a competitive starting pay of $60-63 per hour (DOE) and a supportive work environment focused on resident-centered care.

You will evaluate residents, design treatment plans, supervise PTAs, document progress, and collaborate with an interdisciplinary team to achieve optimal outcomes.

Qualifications

• Current and unrestricted California PT license required.
• Strong clinical judgment and documentation skills.
• Excellent communication and interpersonal abilities.
• Ability to motivate and engage residents throughout rehabilitation.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals.
• Provide skilled, evidence-based physical therapy treatments in accordance with the California State Practice Act.
• Supervise Physical Therapist Assistants (PTAs) and support staff to ensure continuity of care.
• Maintain timely and accurate documentation, including daily treatment notes and progress reports.
• Collaborate with nursing and interdisciplinary teams; participate in care planning meetings.
