Job summary

Highland Chateau Health + Rehabilitation is hiring a PRN In-House Physical Therapist (PT). The PT will assess referred patients requiring rehabilitation, provide direct patient care, and establish a treatment program under the physician's direction.

Responsibilities include setting goals, documenting progress, educating patients and families about the plan of care, and collaborating with all disciplines to deliver patient-centered treatment. MN license and geriatrics experience are preferred.

Qualifications

• Graduated from an accredited Physical Therapy program.
• MN license and registration required.
• Experience in long-term/geriatric care preferred.
• Proficient with EMR documentation (evaluations, progress notes, recertifications).
• Collaborates with all disciplines to provide patient-centered care.
• Maintains confidentiality & HIPAA.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals aligned to patient capacity under physician direction.
• Document patient condition, educate patients and families about the care plan, and set realistic goals.

Highland Chateau Health + Rehabilitation is hiring for a PRN In-House Physical Therapist (PT) candidates to become a part of our team!

Responsibilities Include:

• The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• The Physical Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• The Physical Therapist will set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families with regard to the plan of care.

Qualifications/Requirements:

• Graduated from an accredited Physical Therapy college program or university
• Valid professional license and registration for the state of MN
• Experience with long-term care and/or geriatric care is preferred but not limited to
• Well-versed with EMR documentation Evaluations, weekly progress notes, recertification, and discharge documentation
• Collaborates with all disciplines to provide the best patient-centered treatment and care
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Adhere to scheduled hours and demonstrate professionalism at all times
• Perform all other duties assigned by the Director of Rehabilitation Compliance with confidentiality & HIPAA
