Job description

The Staff Physical Therapist evaluates and treats patients/residents, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. Provides Physical Therapy services including treatment planning & implementation, and discharge planning. Tests patient’s physical abilities and analyzes rehabilitation goals; consults with the rehab team to coordinate the most effective physical therapy program.

Organizational Relationships:

Supervises: Responsible for the care delivered by PTAs and Rehab Technicians.

- Evaluate patients/residents within 24 hours of physician referral.

- Develop effective treatment plans and obtain approval from the referring physician; treat patients/residents according to the physician’s treatment plan.

- Communicate regularly with supervisors, other rehabilitation team members, and nursing staff regarding treatment plans, progress, and prognosis.

- Record evaluations, daily treatment notes, weekly progress notes, 14-day progress reports to the physician, and discharge summaries per policy and procedure.

- Instruct patients'/residents’ families or nursing staff in maintenance programs.

- Secure necessary durable medical equipment such as orthoses, walkers, canes, elevated toilet seats, and grab bars to facilitate independence in mobility.

- Submit logs and billing information timely.

- Report any issues with equipment or facilities to the appropriate supervisor immediately.

- Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board guidelines.

May perform other duties as assigned.

Qualifications:

Licensure/Certification: Bachelor's degree (or foreign equivalent) in Physical Therapy.
